STEPS:

1. Select the Witness screen from the Client Information folder on the Site Plan.   View screen shot.
2.  The Witness screen allows for the entry of witness contact information as well as a description of what the witness observed.

3.   Ensure the desired client is showing in the Client Header screen, and click the Add New button.  View screen shot.

4.  Using the Tab key on your keyboard to move between the fields, enter the following information in the appropriate fields:
  • Last Name
  • First Name
  • Address
  • Phone
  • City
  • State
  • Zip Code

5.  Enter a description of what the witness observed in the Witnessed field.   View screen shot.
6.  Check the needed box if a statement and/or questionnaire is needed from the witness.  View screen shot.
7.  The date the WITSTD1 letter with the written questionnaire was sent appears in the Request 1 Date field.   When the WITSTD1 letter has been sent (prompted by Daily Tickler), ticklers will be set in the Daily Tickler and on the Daily Worksheet.  In addition, an entry will be made in the Client Activity Log.  View screen shot.
8.  Check the Received box when a recorded statement or written questionnaire has been received.   View screen shot.
9.  Repeat the above steps to add additional witnesses.
10. Use the Previous and Next navigation buttons to move between records.    View screen shot.
11.  To delete a witness, ensure that the desired witness information is showing on the Witness screen, and click the Delete button.  You will be prompted for a confirmation of your actions.   View screen shot.
